Is there a way to turn a document into a template?

I have a document in word that I print and use. I work in a medical office
and write in the spaces when patients call in. Is there a way to open this
document in word and type in a patient name, etc. without lines moving. In
other words, can I fill in the blanks and then print. When I try this now,
the characters move because new charters were added. I would like to just
tab to each field and fill in.

Hello,

I need to know where do I can buy MS Word 2003? TheGUYSoftware in
Bellingham that they told me that they do not have MS WORD 2003. I need a
low-price for MS WORD 2003 software because table needs working with Outlook
2003 which I bought Outlook 2003. I need MS Word 2003 which my existing
Word 2002 by MS Works 2002. Could I would have grant of this Word 2003?
