for some reason, my internet explorer and windows

Right click the taskbar and go to properties. there is an
option somewhere about grouping similar tasks. Make sure
it is set to a comfortable number, like 5 or more so as
your windows will not merge together.

Right click the Notification Tray/Properties/Uncheck Group Similar Taskbar
Buttons.

Change number of windows that are open before XP will start grouping them on
the Taskbar: http://www.dougknox.com/xp/scripts_desc/xp_taskbargrouping.htm

In your registry check if BrowseNewProcess is set to 'yes'

[HKEY_LOCAL_MACHINE\SOFTWARE\Microsoft\Windows\CurrentVersion\Explorer\BrowseNewProcess]
"BrowseNewProcess"="yes"
