index format and page numbering

Two questions:

When creating an index, all the listings in the index contain different
formats. I have to manually go through each listing and change to the
standard style. Is there a way for Word to ignore other styles and just apply
the index style without applying bold, italics, etc.?

When creating an index, Word recalculates the page numbers and changes page
numbers I'd like to keep. For example, page 3 should be numbered "page 1" but
when the index is created, it becomes "page 3". Once the page numbers have
been changed by word, the index also picks up the new numbers. Is there a way
to prevent this?

The index will pick up any direct formatting applied to the XE field. If you
don't want any direct formatting, select the XE field and press
Ctrl+Spacebar.
