I am missing a send receive button

Outlook no longer displays the Send/Receive button along the top panel of the
main Outlook screen. The button flashes momentarily after Outlook loads, but
the button goes away.



Also, when I start a new message, there is an Accounts button on the message
pane. When I click this, I get a Microsoft Word error message, saying "Unable
to complete the operation. A fatal error has occurred in Outlook."
