how to keep a locked form from displaying markups on opening.

I have a form that allows users to tab from field to field and enter text or
select from a drop down menu. when I finished creating it, i locked it and
saved it. Everytime i open it now, it displays on the Reviewer Toolbar
"final showing markup" and shows the older format that was deleted. I tried
selecting "accept all changes" but can't seem to get rid of it.

Make sure change tracking is not on.

Select everything except the last paragraph mark. copy/paste into a new
document. Adjust the margins, headers, footers as necessary. Save, close,
re-open. Is this new document "better behaved"?
