how to create a form with calculations

I want to creat a simple calculation form and can't figure it out thru Front
Page.

If I set it up in excel it would read like the sample below. The values
would change everytime you use it the calculator.

1st field: 31
2nd field - 25
Sub Total 6
Add 10
final value 16

I appreciate any help on how to accomplish this.
