How to add fields for SkypeID and Facebook account in Outlook Contact form?

It's not all that hard to do - tools, forms, design a form or when a new
form is open, from its tools menu, design this form.

Amazing! Outlook is so rich and complex that I learn something new
all the time, and that is without upgrading to a new version!

So now I have to figure out how to use my newly designed Contact form
as the default Contact form for new entries. I also have to figure
out how to apply this form for all my 1000+ existing Outlook Contact
entries. More "learning."
