How do you remove surprise password authorization to web page.

Mysteriously, a password authorization box comes up one just one page in one
of my web sites - I didn't set it and I don't want it. How do you remove
password authorization on one web page - no database.
 
Does the page call a file, such as an image file from a password protected portion of the web??

By any chance is the page a word document?
