How do i turn my database into multiple documents?

I have a databae which is a list of 17 employers with ph numbers and fax
numbers. I also have word documnet which is a fax cover sheet. How do I
insert information from my database and create 17 individual word files with
data from one employer in one file, the next in the second file. Please help
need to get this done tonight!

In short you do a mail merge of the data to the Word document. You
need to have a copy of the form in Word that has the table's fields where
you want them.

There are a lot of details I have left out because they depend on
details specific to your application.
