I am creating chapter documents in word, with Heading 1 as the chapter heading.
I have formatted the chapter numbers 1,2,3, etc.. I don't know how to
format the heading numbers to include chapter numbers- is that required?

Anyhoo, I have three chapters complete and want to start playing with the
index and table of contents, and I'm stumpped....

Required by whom? It depends on the book/publisher/content/etc. If you
want the heading numbers to include chapter numbers, Word calls that outline
numbering and you should see the instructions here:
http://shaunakelly.com/word/numbering/OutlineNumbering.html

If you like experimenting, you can just start with the Insert | Index and
Tables and mess with the dialog until you get something you like.
(experiment on a COPY of your book)

Otherwise, there's a large collection of useful links re writing a book in
Word at this page--if you skim the contents you will see a Tables of
Contents section that has links about index and TOC.

First, look at the links that Suzanne gave you. Then take a look at
