I would like to make a form that looks similar to the following but have not
been able to do it. Please let me know how.

I, _______________ would like to ___________ on __/__/____. I want to be
able to set formats and when a person opens this file, he/she can tab through
it and fill it in.

Please let me know how i can set this up. Thank you.
