help creating a form please!

I have fought with word and outlook until I am ready to go back to the
copier and paper and forget it. I want to create a form that I can email to
my teachers, they fill in and email back to me. I created a form in word
but it saves as a template. Basically, I don't know what to do. Can
anybody tell me how to create a form that I can email and it can be filled
in and returned to me?

Some basics: You cannot create a mailable form with Word. You need to do that with Outlook. However, exchanging Outlook forms by email only really works when both sender and recipient are using Outlook and the sender has some way to get the form design template to the recipient.

So, are your teachers all using Outlook? If not, then you might be better off using a Word document. It sounds like you're already 95% there. Now that you've saved your Word document as a template, you can create a new document using that template by using the File | New command in Word and choosing your template. After you fill in any needed information, save it and send it as an attachment in an email message.
