How to embed a form into an email message?

Hi everyone,

Currently we have a form (MS Word) which has text box, check box, radio
button,..., etc for users to enter information. Whenever we want users to
fill out this form, we attache this MS Word file and email to them by
outlook. Now we would like to integrate it into Outlook (we are using Outlook
2000), so that the form can be embeded into an email message and users can
enter information in the email message directly without having any
attachment. Actually I have no idea on how to do this..... Does it deal with
creating an Outlook form? I have no idea and I would be grateful if you can
suggest any solution. Thanks in advance!!

It is not possible to embed a Word form in an Outlook message or Outlook
message form. You would have to create an Outlook custom form instead --
from scratch, nothing being reusable from the Word form. If you want to
