Global Address List and mail merge

How can I set up a mail merge in Outlook 2003 using the Global Address
List?

Export the GAL using some third party tool. Presumably, Mail Merge doesn't
work with the GAL because internal users don't care if their email address
gets shared with everyone else. Of course, this logic gets broken when you
consider external contacts that get added to the GAL, but, well, that's that.
