Forms in FrontPage

I have designed forms in FrontPage for employees to use to request time off
etc. The forms are submitted to an email address. Due to security issues on
the server where my webpage is housed, it will not allow the sender's email
address to appear as the sender of the email. One of the fields in my form
is for the user's name. Is there a way to have this field included in the
'subject' line of the email?

Set the form to use a field on the form where the user would enter their email address under From
Properties, Reply to.
