form to email problem

I have a Front Page 2003 website submitting the contents of a form to an
email address on my domain. This is then set in my domain control panel to
redirect to my personal email address. I cannot make it work and have never
actually received an email from the form.

My server admin tells me that my account is correctly set up for email
transfer but that their windows-based server requires a "from address" in the
form html. Since I am using standard FrontPage form properties (with email
results set to the correct email adress only, and the sender's email address
picked up from the correct field), I do not know what else to do. If I set
the form property results to send the data to a file, the correct form fields
and information are delivered.

Does anyone know how to set a "from address" under such circumstances?

The From address is set up in the FrontPage extensions configuration -
normally done by the host.

Thank you for your advice. I will go back to the server administrator and try
again. I appreciate your help.
