FORMS FP2003 are not WORKING

Hi
I am trying to post my form using FP2003. I dont know why on the FORM header
when it is running on the IE, it shows [FrontPage File Upload Component] or
the name of one of the component on the FORM.

Besides this, the form is not working after press SUBMIT. I am not sure why?

It is telling me that there no possible to run FP extensions for this page,
but the server has that enabled.

You are attempting to use the FP 2003 File Upload which requires
- A Windows Server running the FP 2002 SE
- A host that allows File Upload permissions
- Permissions set per http://support.microsoft.com/default.aspx?scid=kb;en-us;299763&Product=fp2002
