form results won't email

I seem to be able to make a form work ok unless I add the option of emailing
the results to me. Then it doesn't work. Does this mean my host doesn't have
that option set up for my site? Or am I doing something wrong in FP?

This form without email results works ok:

but this one with email (supposedly) doesn't work:
http://www.cloud9coaching.com/_vti_bin/shtml.dll/new_form_04.htm
Actually, it sort of works, but it adds my request as text at the bottom
instead of sending me an email. Any fix? TIA

Try right clicking somewhere inside the form and clicking "form properties".

Click "send to" and fill in your e-mail address.

Yes, it mean your host hasn't configured the extensions to process email.
