form problem

I published the custom form in public folder.When users want to change
outlook views and select the custom fields ( used in P.2 page) from field
chooser they can not add the field on to the right pane and outlook gives
an error message as "The operation was canceled".

any idea ?

Did you add the fields to the public folder? Publishing the form doesn't do
that. Try adding them manually using the folder's Field Chooser.
