form not visible in preview mode

Is there a setting that will allow the form to be visible in the Outlook
Preview Pane. It currently looks like a blank email & folks are deleting it.
Also, is there a way to keep this form at the bottom of an email so that it
remains part of an ongoing email trail.

No. The preview pane cannot display custom form layouts, A solution might be to put some default text in the message body telling users that they need to open the item.
