Form not showing as form for other user

I have created several custom forms that I have distributed to other users.
They work fine but for one user. It just looks like an email when she opens.

Any suggestions on what the problem could be.

Distributed how? For these message forms to work properly, they need to be published to the Organizational Forms library or to each user's Personal Forms library.
