form info that sends you to a page depending on the info inserted?

I would like to know PLEASE!... how do i create info that when i click on a
field that requires digits, how can i make these digits send the visiter to a
certain page that depends on what they type in. so for example, if i wanted
to go to page A i had to type in 676767 in the field, but how can i configure
this so that i can go to many different pages depending on what i insert. so
if i wanted to go to page B i had to type in 676768 for example. Please help
me people. i would be most grateful.

Write a solution in Javascript that takes the input evaluates it and
redirects to the correct page.
