Form in Word 2003

I am working in Microsoft word 2003 and would like to convert a document to a
form. I have a document that I would like to be an interactive form. I
believe I almost have it, but I can’t stop the lines from moving. Also, if
there’s a simplified way, please let me know.

The easiest way to obtain and to control the layout you want is to use a
table. Add table borders where you want lines to appear (the column width
determines the line length). For example, type "Name" in one cell without
borders and insert the related text form field in a cell with a bottom border.
