Form error

You've made the submit button a mailto link and that will not work, as the form is automatically
mailed when submit is clicked.
Switch to code view and remove this line:
<a title="Email this form to Chuck" href="mailto:[email protected]?subject=TEC Chair response">

Which is right above this:
<INPUT TYPE=SUBMIT VALUE="Submit Form"></a>

Save and republish.
