Did you create it by yourself? If yes, then reason could be that you had
not
saved the file with proper file type. If the file name is appearing
something
like "disablerun.reg" with a Notepad icon then open the file again in
Notepad. Click File menu and click Save As. Type a new name e.g.
disablerun.reg for the same file and from the "Save as type" drop down
menu
choose "All Files" and click Save button.

As said by Ramesh, right-click on you Registry file and if there is no
Merge
option then it is sure that the file has not a proper association.
Actually,
this is a notepad file with .reg as the first name. You can still use this
file by renaming it.
Click Tools - Folder Options - click View tab and uncheck "Hide extension
for known File type". Rename the file e.g. disablerun.reg and press enter.
See a change in icon shape. Later, make a check mark again on the same
Hide
Extension option.
