faxing

I am can't figure out how to attach the scanned document I
want to fax to the fax cover sheet. I don't see a prompt
in fax wizard or an attachment box to click.
 
Correct. The Send Fax Wizard does not support faxing attachments.
Print to the Fax Printer.
