Failed redundancy

I have Server 2K spk 4 with all patches. My raid 5 has 3 volumes. Only one
volume keeps falling off, which is J drive. It show in the event viewer that
there are I/O errors.
I replaced oun hard drive. With in minutes another drive gave me online
errors. I called adaptec and the said it could'nt be there card. I was told
that the OS sometimes causes I/O problems. How do I trace the problem? If
it's software or hardware problem.
Could a file or folder cause this problem?
I need help.

Rev, probabily its a hardware issue; operating systems does not cause this
kind of problem.
You can find sereveral problems, like slow physicall disk access, HD
damaged, controller problems, etc.

Would the problem just affect one volume? And would all volumes experience
this slow down?
