edit outlook form

I am trying to edit a form that I previously created by adding some
fields...I added them to the compose page and the read page but they do not
stay.

Saving or publishing? Forms for reuse need to be published.

Are you incrementing the version number on the (Properties) page each time you publish? This will help you keep track of which version of the form you're actually seeing.

Try cleaning out the cache. It is probably best to locate the FORMS folder
in Windows Explorer (close Outlook first) and delete any forms in that
folder. Also, if your frmcache.dat file is too large, it is best to let
Outlook recreate it. Change the dat to a different extension, then reopen
Outlook.
