Duplicate Read Receipts

I have a user that gets duplicate read receipts upon opening of the email
recipient. This does not occur with each recipient or each email. I have
asked him to remove all rules and filters and restart outlook. This did work
but the problem is random and I want to make sure the duplicate read receipts
will no longer occur. Any ideas of why this happens?
