Duplicate Icon on Quick Launch Toolbar (QLT)

I have an Outlook shortcut on my QLT which, when I place my cursor over it,
shows the comment, "Microsoft Office Outlook 2003 - Send and receive e-mail;
manage your schedule, contacts, and tasks; and record your activities by
using Microsoft Office Outlook" (i.e. the same comment as that on the
standard "Start"/"All Programs" menu from whence I dragged a copy to produce
the shortcut on the QLT.) This QLT outlook icon I use regularly.

Occasionally, however, for reasons I cannot fathom - it seems quite erratic
- but probably isn't as there must be some logic behind it), another shortcut
icon is added to the QLT. The comment on this one is much shorter and, from
memory, just states, "Microsoft Office Outlook 2003".

This happens on both my desktop and my laptop.

While not a great bugbear (I just delete this second one on the infrequent
occasions that it appears) it's really annoying and has been going on for
many months. Any solutions to stop this happening?
 
That would depend on your configuration. Are these machines connected to a
domain with roaming profiles enabled?

No. They're not connect to a domain with roaming profiles enabled. Both are
set to "local" profiles.
