Duplicate Outlook icon appears in Quick Launch bar

Occasionally a duplicate Outlook icon appears in my Quick Launch bar. What
causes that and how can I prevent it? I'm running Outlook 2003 under Windows
XP Pro SP3.

Office updates will cause the Quicklaunch icon for Outlook to reappears.
Just delete it it you didn't have it there before.
 
Deleting the extra one is what I have been doing. How come this doesn't
happen on my other computers with similar configurations?

A car driven over a road full of nails gets a flat. Other cars don't. Shit
happens. I don't know the code for the updates to know when or which ones
will regenerate the Quicklaunch toolbar button. I do know there are nails
on those "update" roads.
