Documents is listed on the Start Menu, but it is empty.

Under (Task Bar Properties > Start Menu > Customize) I've selected
Documents (Display as a menu). Documents is listed on the Start Menu,
but it is empty. The reason that is is empty is that I deleted the
shortcut from my desktop which seems to have broken this (link/
association/shortcut).

How do I recreate the (link/association/shortcut) on the Start Menu?

Go to Start > All Programs > Accessories and right-click
on the Command Prompt and choose "Run as administrator".
Type in: SFC /SCANNOW and press your Enter key.

If that does not work, try repairing your system. Boot from your Windows
Vista DVD, select the System Repair Option > on the setup screen, select
'Repair Computer > Select your installation of Windows > select Startup
Repair option and follow instructions.
