Dial up window appears on boot up

A few weeks ago the modem dial up window began to appear
upon boot up of the sytem. This annoyance continues but
not every time I boot up. I'm concerned this may be a
bug, a vehicle some hacker has implanted to secure my
info. Appreciate any advice on how to fix.. I've tried
the options list on Explorer and Outlook Express and i
don't see any setting that would cause this problem.
