Custom Outlook Form

I have created an outlook form with text boxes, labels, check boxes etc...I
copied the exact form from the compose page to the edit read page however
when I send the form only the values from the "Notes" section is displayed to
the recipient? any suggestions?

Sorry, My problem was with the check boxes, the value on the edit page won't
display...

Did you set the text box value to "Yes/No ?

Did you click the Edit Read Page button and put a checkbox on the read layout that is bound to the same property as the check box on the control layout?
