custom message form-sends as a blank

I have designed a custom message form, but am having problems when sending
it. The form has various comboboxes, checkboxes, and a message field for
inserting attachments. When I complete the form (including an attachment)
and send it to myself to check the form, all I get is the message box with
the attachment.
I am completely new to designing forms and would appreciate any ideas!

Umm...No. I wasn't aware of that. How do I go about doing that?

Did you create a separate read layout for the form? Message forms, by
default, have separate compose and read layouts.

Go to www.slipstick.com, search using the term TNEF, the first entry of the
resultset should tell you what you need to know.
