When a user creates a Time-Off request that is located in the Organization
Folder (which I have created/published) and the manager approves it and then
deletes the message, he is unable to open it in the Deleted Items folder.

He gets this message...
The custom form could not be opened. Outlook will use an Outlook form
instead. The form required to view this message cannot be displayed.
Contact your administrator.

It is published in the Organization folder on the server and it was created
in OL2003

Where is the form published? What version of OUtlook?

That sounds like Outlook 2003's forms cache corruption problem. The first thing to try is applying SP1, then clearing the cache. See http://www.outlookcode.com/d/ol2003problems.htm
