Cross References Act/Appear Differently in different documents

First off, we do not use the "built-in" styles since we have discovered this
is a disaster waiting to happen when we modify them. Instead, we have
defined our own set of headings, lists, etc. This has worked out very well
for us.

When using cross referencing, however, we occasionally run in to a problem.
It works fine 95% of the time; but it's that 5% that causes us problems (like
right now!) Usually, when I insert a cross reference using "Numbered Items",
all the headings and lists show up (and just the item # & text) and I can
easiy select what I need to insert. However, on rare occasions, I will see
items listed with the page numbers next to them in addition to the # & text.

If I select from the list w/"Paragraph number", the paragraph number shows
up as "0"; w/"Paragraph text", it shows up with the text + page number. I
don't want the page #, just the item # and/or text (depending on what I
select)

In all cases the "Reference type" is "Numbered item" and only the "Insert as
hyperlink" box is checked.

Like I said, most of the time everything is fine, it's just the rare times
that we have problems. Any ideas about what is happening?

The problem is most likely that your have pressed Return while the insertion
point was in the start of an already Ref-bookmarked paragraph - this results
in the new paragraph being included in the bookmark. See the demo below

Unfortunately, that's not the problem. I'll have to come up with another way
to explain it since I must not have been clear. This happens only rarely and
it happens no matter where you are in the document when you attempt to insert
any cross reference. Right now, though, I need to finisih up a
delierable...I will try to find time tomorrow to update you with more
information.

Since I didn't have any more time to fiddle with it, I just created a new
document and started over. *sigh*
