Creating forms?

Dazed and Confused

How do you create a custom from from a blank template? I
can only seem to use a previously defined template then
use it... and that really limits me in what I can/cannot
do. :-(

I have yet to find a blank form in outlook. I have always

had to use one of the standard templates but I am able to
edit to how I want to use it.
