Creating a template with Headers and Footers

I have created a template for our company’s letterhead. We have an image for
the Header and another (different) image for the footer. I also need to
create a 2nd and 3rd page letterhead template where the Header image is only
located on the 1st page, but the image for the footer is located on each
page. I am not having any luck!! The Header continues to show up on either
every page or every other page. Any help you can provide would be greatly
appreciated!!
