converting Word document into a template

I would like to convert a document that was created in Word, to a template.
This is an agency form that we complete on clients. I have tried numerous
time to save it as a template. However, each time I try to enter client data
the form shifts.
I am using Word 2003.

I would like to be able to print client data on this form, without
everything shifting.
