I've been using Switch http://nch.com.au/switch/faq.html#ACA
However the other approach is to try Real Alternative Media Player Classic

Real Alternative/Media Player Classic does _not_ perform any format
conversions. It simply renders the stream as an audio feed out to your
speakers.
We've been over this time and time again. Please search backwards to
find recommended utilities that can grab and convert real streams.
Try it; you'll like it.

I think that's why plodda called it 'the other approach' to listen to real
audio without having to use the Real Player.
