Since the original is now buried on Page 6...
To repeat, when I choose the Connect with a broadband
connection that requires a username and password from the
New Connection Wizard, it creates a Dial-up connection
that has a status of Unavailable - device missing.
All the hidden ports are "functioning normally" according
to Device Manager. Any suggestions before I do a complete
wipe of my harddrive and do a clean install?

The WAN Miniport is enabled and XP says it is working
properly. XP is using its default driver, dated July 2001.
There are 3 other WAN Miniports, IP, L2TP, and PPTP. All
are "working properly." Should I try uninstalling the
PPPoE port and letting it reinstall itself or is there a
way to get newer drivers for it without having the XP
machine online?

Check you WAN Miniport (PPPOE) in Device Manager to verify that it is
enabled and working
My Computer --> Properties --> Hardware --> Device Manager -->
View --> Show Hidden Devices
Make sure that the PPPOE Miniport is not banged out or disabled.
If it is disabled, try and enable it and see if it works
