checking a slow pc

My pc is running extremely slow on and off line, even though I cleaned and
defragmented all and everything. I think there is a lot running in
background. Used pc bug doctor and others but although they find many
problems, they wont fix, less you purchase. Is there a free pc-doc download
that will also fix?
