Checkboxes & lots of them

I am creating a database which will be used as a backend to an internal
website.

One of the table will hold data that our customers will fill in. This is
more like a questionarrie and about 6-7 questions are multiple choice i.e.
checkboxes.

Does anyone have any suggestions as to how to store those responses can be
stored in the database? For example one of the questions have about 16
responses? Shall I create a seperate table for each of the multiple choice
questions?
