Tables related or no?

I'm trying to setup a training database based on a survey that the supvs
take. When they're answering the survey, the questions, the supvs info,
responses submitted, and response choices are all different tables. When I
create a new question it may require two training classes or a training class
may be required for different questions. I tried a multiple to multiple link
and having a table between the training table and the possible responses
table, but it didn't work out. Am I going about this the wrong way? I just
want to create the question and select the different training that is
required for a "yes" response. Thanks and sorry if this is confusing.

will the db hold only one survey or do you need to store records for
multiple surveys? (it will make a difference to the table structure)

It is indeed confusing. We can't see your screen!

What are the structures of your tables, and how are they related? Have you
looked at Duane Hookum's excellent At Your Survey sample database:

In addition to Duane Hookom's "At Your Survey" database (that John
mentioned), you also might want to look at another small sample database on
my website called "TrainingRegistration.mdb". This was my solution for a
general purpose training registration database that I have adapted for a
variety of circumstances. You can find it here:
http://www.rogersaccesslibrary.com/download3.asp?SampleName=TrainingRegistration.mdb

It is in Access 97 format, but can be upgraded.
