Captions and Table of Contents

How do I avoid figures/maps actually showing up in the Table of Contents?
When I prepare a list of maps for instance using the Reference/Index and
Tables/Maps option, sometimes one or two maps get inserted into the TOC.

I go to the Caption and make sure that is has the Caption style and the map
has the Normal style yet it still shows up.

I have heard of formatting getting "stuck" in paragraph markers but even
when I delete any of those before or after the caption and map, it still
shows up in the TOC. Help!

Unless you're being clever with the field format, the TOC simply picks up
all paragraphs that have an outline level matching whatever you've selected
(heading 1 only, heading 1 - 3, etc). Any paragraph that shows up in the
TOC must have a matching outline level. You can check by selecting the
paragraph in question and displaying the Format > Paragraph dialog: the
outline level is displayed at top right.
 
Jezebel has explained that the problem is the outline level needs to be
reset.

It's possible that using the Document Map may have changed your outline
level for captions--that's explained here, with some quicker fixes than
manual Format | Paragraph at the end:
