Blind Copy on replied/forwarded mails

Is it standard that when a mail is replied or forwarded, if anyone was bcc on
the original, they get bcc on the reply or forward?

I received a mail, replied, and then got a reply from someone I didn't know I
had sent a mail to.

I then forwarded the same mail later on in the conversation, to another
person, and got an out of office reply (from the same person as above) who I
didn't realise I had mailed.

Whilst I understand the need/desire for sending with blind copies, and
receiving mails without the knowledge of who else has received it, I appear
to be sending mails to people I do not intend to or even know about. Is this
right?

Replies don't go to BCC'd people since your machine doesn't know about them.
As far as the odd recipients... could someone (or something) be forwarding
the messages automatically to other people?

I don't know. How would I tell? As far as I am aware I have not set any
preferences to indicate this.
It sounds a bit of a "conspiracy theory" but is it possible that all outgoing
mails go via my boss's e-mail for vetting?

It could but that's a bit paranoid. You might want to check with your mail
administrator to see if there's any sort of automatic routing
