Blank Documents contain text

For the past while, whenever I start a new blank document in Microsoft Word, the new document comes up pre-loaded with the complete mailing address of someone who I had e-mailed several months ago. I have no idea how it got that way, and I have no idea how to get rid of it.

It's beginning to drive me nuts! Each time I start a new document I have to erase this person's contact information. Does anyone know what I can do to remedy this?
 
Thank you for your help. I'm usually a Mac user and this problem has never
come up. What I did was to go into "New Word Document" and when the window
appeared with the choices, I deleted the icon that said "Blank Document" and
it resolved the problem. I couldn't find the .dot file, so if someone else
has that problem, they can try it my way.

I wouldn't have known where to start had it not been for you. Again,
appreciate the help.
