AutoText entries not formated correctly in header/footer

When inserting AutoText for a Header/Footer for Word 2003 document the text
is not inserted correctly --- the text format contains squares and what
appears to be the script for entering the text.

I already tried uninstalling/re-installed Office and this did not fix the
problem.

You're probably seeing the field codes. Press Alt+F9 to toggle the display
to field results.

I've tried that and it didn't work. It is showing something similar to the
field code but has squares around the words like DATE and AUTHOR.

In that case, I'll guess that the AutoText entries in your Normal.dot
template are corrupted in some way, so that the field delimiters aren't
being recognized. Normally they appear as bold curly braces { } when field
codes are displayed. Word usually displays boxes when it doesn't understand
the characters.

Go to http://word.mvps.org/FAQs/AppErrors/BlankDocNotBlank.htm and use the
procedure under "Creating a new Normal.dot from scratch" to build a new
Normal.dot template with the default set of AutoText entries. Before you do
anything, read to section about "Salvaging your customizations", as well as
http://word.mvps.org/FAQs/General/FilesToBackup.htm.
