Attachment icons

When I attach a document to an e-mail - sometimes the attachment icon appears
in the body of the e-mail text (ie: wherever the cursor is when I click
attach) and sometimes it appears in the attachment title bar in the address
section at the top of the e-mail. I always want the attached doc to appear in
the title bar. How do I achieve this?

It you want the attachment always to appear on a line of its own and not in
the body, always use HTML or Plain Text for the message format. Using Rich
Text will appear to insert the attachment within the message body.
